Advanced Care
Our Advanced Care program is designed to provide support to those residents needing more physical/medical assistance in a caring, home-like environment. Services are provided within a cozy neighborhood of private and semi-private studio apartments. We are able to provide assistance with transfers, meal set up, adaptive equipment, toileting, and other physical needs. Our staff is highly trained in areas such as Medication Management, Challenging Behaviors, First Aid, Fire Safety, Nutrition/Dietary and Safe Transfer Techniques.
Additionally, Liberty Village offers daily activities, community outings, 3 homemade meals per day, housekeeping/laundry service and assistance with medications or activities of daily living. Your service plan will be designed to fit your needs. We will work with you to accommodate any changes in physical or cognitive status and allow “aging in place.”
Memory Care
Liberty Village’s Memory Care is designed to assist residents living with Dementia in a caring, home-like environment. Services are provided within a cozy neighborhood of private and semi-private studio apartments. Our staff is highly trained in concepts such as Medication Management, Challenging Behaviors, First Aid, Fire Safety and Nutrition/Dietary. Liberty Village has an interactive alarm system for those residents who may be at risk for wandering. While the entire unit is not locked, those residents at risk are monitored and staff notified if they would attempt to leave the unit. Additionally, Liberty Village offers daily activities, community outings, 3 homemade meals per day, housekeeping/laundry service and assistance with medications or activities of daily living. Your service plan will be designed to fit your needs. We will work with you to accommodate any changes in physical or cognitive status and allow “aging in place.”
Life Enrichment & Wellness Program
At Liberty Village, we are dedicated to improving the quality of life of our residents. We focus on the physical, mental, emotional and spiritual welfare of the entire resident family.
- Foot Care Nurse Visits
- Blood Pressure and Weight Clinics (performed by our nurses)

Nursing Services/Medical Management
At Liberty Village, we believe in charging our residents only for what they need and use. An individual assessment will be conducted to determine what Level of Care is necessary or desired: We offer the following services for additional fees:

- One Person Transfers

- Morning & PM Setups
- Toileting Assistance
- Doctor ordered Treatments & Therapy

- Doctor Ordered Special Diets
- Tray Service (Sick/Doctor ordered)
- Transportation with Staff Escort (Note: Resident pays 100% transportation cost)
- Additional Meal (family & friends)
- General RN/LPN professional nursing care (Services not listed or outside the scope of listed service costs)
- Skilled RN/LPN professional nursing care (e.g. wound care, diabetic nail care, etc.)
- Nursing Assistant care (for services not listed)
